Guelph Transit Services Subsidized Bus Pass for Adults with Disabilities Terms and Conditions |

1. Eligibility is determined by 4 factors:
The total combined income of the applicant before deductions. (Supporting documentation must be provided, for example: Revenue Canada income tax return, pay stub cheque stub from government assistance such as Ontario Disability Support Program/Canada Pension for Persons with Disabilities, insurance benefits, etc.)
Applicant must be between 18 - 65 years of age.
Applicant has either a permanent or temporary disability
Availability of funds
2. Conditions:
The Subsidized Bus pass is only issued for use on the regular Transit service and does NOT apply to Guelph Mobility service
Cost of bys pass is $30.00 (regular adult pass is $50.00)
Applicant must present approval letter in order to receive the subsidized rate.
to purchase subsequent passes or expired bus passes, personal identification must be presented.
Each month a list of approved applicants will be given to Centre Smoke and Gift Shop at the Guelph Centre.
Subsidized passes will be sold only at Centre Smoke and Gift Shop at the Guelph Centre on the lower level.
3. Application Process:
Individuals within to apply for a Subsidized Transit Bus Pass must complete an application (see hyperlink below for application form).
Complete application must include supporting documentation to verify income.
Forms will be accepted, reviewed and processed on a monthly basis and applicants will be informed by letter.
Applications will be processed on a first come, first serve basis.
Approval will be given on a 3 month basis, unless otherwise specified. NOTE: Some individuals may not be eligible for a discounted pass for an extended period, i.e. person may only need the service while recovering from an injury, OR some individuals may be approved for a periods of up o one year if there is no chance their disability or situation will change.
Applicants will be reviewed yearly to ensure financial status and disability are unchanged
If you are approved and DO NOT purchase your bus pass, your application will be withdrawn and another person will be granted approval.
